The Care Partner provides direct, hands-on care to residents, assisting with activities of daily living (ADLs) such as bathing, dressing, eating, and mobility. They also monitor residents’ health, communicate with families, and offer companionship, ensuring that each resident’s needs are met with dignity and respect.
The Private Duty Aid provides one-on-one personal care and support to residents in their private residences. This role offers assistance with activities of daily living, including bathing, grooming, and meal preparation, ensuring that residents receive individualized care tailored to their specific needs.
The Wellness Nurse/RN is responsible for overseeing the health and wellness of residents. This role involves conducting assessments, creating care plans, administering medications, and providing education on health management. The Wellness Nurse works closely with the care team to ensure optimal care for residents.
The LPN/LVN provides hands-on nursing care to residents under the supervision of a registered nurse or physician. Responsibilities include administering medications, monitoring vital signs, assisting with medical procedures, and supporting the overall health and well-being of residents. The LPN/LVN is an integral part of the clinical care team, ensuring residents receive compassionate and professional care.
